Your insurance company will issue an EOB for every doctor’s visit or hospital stay. This EOB is a statement explaining your coverage for that individual stay. The EOB gives a detailed breakdown of what was billed, what they paid, and what you may owe the Hospital or Provider.

Out-of-Network simply means Acuity has not signed a contract with your particular insurance provider.

You may receive multiple bills from providers who have seen you while you were under our care. This is dependent upon what type of care you received. Types of bills you may receive include, but are not limited to, Radiologist, Anesthesiologist, etc.

Many times your insurance company will pay the Hospital on your behalf but they often also leave a patient balance. This generally includes deductibles, coinsurance, and/or copayments.

Once you are discharged from the facility, your bill is then generated and sent to your insurance company. Your insurance company then pays your bill and sends an Explanation of Benefits to both you and the Hospital. If you have supplemental or secondary insurance coverage, we will then bill your secondary coverage any outstanding balances prior to billing you.

Please keep in mind, there are instances that you may be required to reach out to your insurance company to clarify any questions they may have, prior to payment of your bill.

The Acuity Specialty Hospital financial assistance program is designed to ensure assistance is provided to patients demonstrating a financial need and to ensure Acuity Specialty Hospital complies with any required Federal or State regulations related to financial assistance. Those eligible for financial assistance will never be billed more than the amounts generally billed to an insured patient.

Medical Records

Health Insurance

Acuity Specialty Hospital accepts Medicare and most commercial insurance plans. Hospitalization insurance information is obtained prior to admission. The Admissions Department works closely with all insurance plans to verify coverage benefits, policy limitations and authorization. The hospital routinely contracts with out-of-network insurance plans on a patient-by-patient basis.  Patients should check with the physicians involved in their care to determine whether they are in or out-of-network.  Using an out-of-network provider could increase the patient’s financial responsibility.  Upon request, we are happy to provide you with an estimate of charges.

For those patients who are not covered by a health plan, financial arrangements will be discussed prior to the patient’s admission.

Acuity Specialty Hospital is committed to empowering patients, in partnership with their care team, to make informed decisions about their healthcare. This includes helping patients understand the cost of their care, as well as financial assistance that may be available.

In compliance with federal and state law, Acuity Specialty Hospital will provide standard chargemaster prices to anyone who requests it. However, the list price of a hospital service is irrelevant, and not the actual amount a patient will pay. The amount a patient pays is based on many factors, including health insurance and other applicable discounts, and the unique needs of the patient.

To help patients plan for their medical care, we provide estimates for many common hospital services. Call 877.228.4893 (option 3 and press 3) Monday through Friday, 8:30 a.m. to 5:00 p.m. to speak with one of our patient financial services representatives.

Annually, Acuity Specialty Hospital formally reviews our hospital prices to ensure they accurately reflect the high quality care we seek to provide. In some cases, our prices are different from other providers when we offer a higher level of care and unique clinical expertise. Overall, Acuity Specialty Hospital works with patients, providers and partners to lower the cost of care through wellness, disease management and quality care.

Medical records hold important information about your health history, diagnoses, conditions and treatment plans; including medications, you received while at our hospital. Our hospital is transitioning to digital medical records, which will enable us to help provide seamless information sharing between your doctors and the hospital. Regardless in what format your medical record is kept, you have the right to access it.

Since your medical record is the property of the hospital, all medical providers involved in your care must first document your condition, treatment, and the care you received before it can be released to you.

Please keep in mind that medical records are confidential and, to protect your information, your written authorization is required before a copy can be released. If you’re age 18 or older, you can request an authorization form from the medical records office. A fee may be required.

Patient Right of Access

You have the right to review and/or request a copy of your protected health information. This includes the right to ask the hospital to transfer a copy to a selected person or other healthcare provider of your choosing. You have a right to access your protected health information for as long as the information is maintained by the hospital.  This rule also extends to a patient’s personal representatives who have the right to make healthcare decisions on the individual’s behalf (power of attorney, legal guardian, parent of a minor, etc.).

Although we make every effort to respond to all requests as quickly as possible, the law allows us up to 30 days to respond.

Invoices for medical records are in compliance with HIPAA and state laws. We’re required to notify you up front of any fees associated with your request and the invoice is this notification. With price options, you’ll be able to decide which is most affordable. You may contact the hospital for any questions you may have.
