How do you make a referral to Acuity Specialty Hospital of Southern New Jersey?
Acuity Specialty Hospital of Southern New Jersey accepts referrals from hospitals, clinicians, rehabilitative centers, skilled nursing facilities, and families. Our hospital is Medicare certified and accepts most insurance plans, as well as workers compensation.
To make a referral, please contact the Admissions Department at 609-835-3655. An Acuity Specialty Hospital Clinical Liaison will conduct an on-site evaluation. Financial assessment of insurance coverage and eligibility is also determined at this time.
All admission determinations will be made and communicated to the referral source. Upon approval of a patient’s admission, communication of clinical information is coordinated by our clinical liaisons along with the admissions department. The medical information is then transferred to the patient’s new chart upon their arrival.

Davita Garfield, MHA / Director of Provider Relations
DaVita joined Acuity Specialty Hospital in 2018. She has over 18 years of healthcare experience and has spent the last 11 of those years in healthcare sales as a clinical liaison and a sales and marketing director. She holds a bachelor’s degree in social work, a master’s degree in health administration and earned her certification in coaching in 2017. DaVita’s number one role is being the mother to her amazing teen daughter, Aniah Rose. She spends her downtime pouring into her family and her girls mentoring program.
Lori Cook, RN / Clinical Liaison
Lori Cook, RN has been a registered nurse for over 25 years. She has experience in multiple areas in the medical field including geriatric, MedSurg, medical sales, field manager for a dialysis lab and pharmaceutical chart auditing. In addition, she has worked as a Clinical Liaison for 12 years, 8 of which have been with Acuity Healthcare.
Kristen Smith, MSW, LSW / Clinical Liaison
Kristen Smith started with Acuity Specialty Hospital in May 2016. She is a Licensed Social Worker who has been a clinical liaison for over 12 years in various different levels of care. She has worked in both sub-acute and acute care settings for over 15 years.
Tina Sheets, MBA / Business Development Specialist
Tina Sheets started with Acuity Specialty Hospital in spring 2016 as our Business Development Specialist. Tina has her MBA with a concentration in Healthcare Administration and brings with her 15+ years of in the field experience.
Greg Harper, CRT, RCP / Lead Clinical Liaison
Greg Harper began his career with Acuity Specialty Hospital in 2009 as a Respiratory Therapist. In addition to being a Certified Respiratory Therapist, Greg also has an A.A.S. In December 2015, Greg transitioned into the role of Clinical Liaison and then took on the role of Admissions Manager. His current role of Lead Clinical Liaison puts him back in the field managing admissions and building relationships with case managers and clinical staff.
Susan Chilkotowsky-Kain / Admissions Manager
Susan has been with Acuity Specialty Hospital since 2009. She began her tenure at ASH as the receptionist, transitioned into the interim HR coordinator position and has been with the Provider Relations Department for 6 years. Susan was given the role of Admissions Manager to make the patient and family transition to LTAC as smooth as possible. In her spare time, Susan can be found with a camera in hand photographing landscapes and wildlife.
